SOYUZ U & U2 (11A511U, 11A511U2) - Vehicle use: Soyuz, Progress, Satellite
Russian orbital launch vehicle. The Soyuz U launch vehicle is derived from the Soviet R-7 intercontinental ballistic missile and produced by the Progress Rocket Space Center. It is comprised of four strap-on boosters (stage 1), a central core stage (stage 2), and an upper stage (stage 3). The Soyuz U variant was used to launch the early Progress resupply vehicles to the ISS. Both the Soyuz U and U2 have been retired and replaced with the Soyuz 2.1a.

PROPULSION VALVE FAILURE
Launch attempt of Soyuz T-10
Approximately 90 seconds before the intended launch, a propellant valve failed, resulting in rocket-grade kerosene (RP-1) leaking out. The fuel ignited and quickly engulfed the launcher. The ground crew commanded the launch escape system to activate, which saved the two cosmonauts.

The Russian Commission determined:
On the basis of analyzing the behavior of the parameters characterizing the operation of the propulsion system the third stage, and results telemetry data it is concluded that reducing consumption of fuel in the gas generator due to contamination tract of its submission. This led to a breach of working conditions and reduce the engine parameters, it shut down on command “Emergency engine shutdown.”
The construction process was reviewed and additional processing controls were implemented.

PROPULSION SYSTEM FAILURE
Launch of Progress MS-04
The Russian Commission concluded the oxidizer tank of the third-stage engine was failed due to the impact of fragments from the failure of the third stage engine. The failure of the third stage engine was found to be due to the failure of the oxidizer pump. Potential causeses for the pump failure were identified as the presence of foreign particles inside the system or the violation of the engine assembly procedures. The root cause was attributed to workmanship. Assembly procedures were reviewed and updated and additional processing controls were implemented.
